Or, one of the nearest relatives (Parents, or husband/wife) can attend instead to complete the procedure with providing the following:

Disability card of the subscriber with the special needs.
Proof of subscriber’s identity.
Proof of deputy’s identity.
Service charge of 5 LYD.

If the subscriber is a minor (who is below the age of 16) then, one of the guardian can attend instead to complete the procedure with providing the following:

Disability card of the subscriber with the special needs.
Proof of guardian identity.
Birth document of the subscriber with the special needs (in case if the guardian was a mother).
Service charge of 5 LYD.
